NurseLink Clinic, operated by Nurselink Corp., is committed to protecting the privacy and security of your protected health information (PHI). We provide secure telehealth services to eligible patients located in Florida, Arizona, and Texas, where our provider is licensed to practice. This Notice of Privacy Practices explains how we may use and disclose your medical information, your rights regarding your health information, and our legal responsibilities under the Health Insurance Portability and Accountability Act (HIPAA).
Your Rights
As a patient, you have the right to:

- File a complaint if you believe your privacy rights have been violated.
How We May Use and Share Your Health Information
We may use and disclose your protected health information (PHI) to provide treatment, coordinate your care, obtain payment for services when applicable, operate our practice, comply with legal obligations, and for other purposes permitted or required by law.

• Legal Requirements – When disclosure is required by federal or state law.
• Public Health and Safety – When permitted or required to protect public health or report certain conditions.
Our Responsibilities
NurseLink Clinic is required by law to maintain the privacy and security of your protected health information. We will notify you promptly if a breach occurs that may have compromised the privacy or security of your information. We must follow the duties and privacy practices described in this Notice and provide you with a copy upon request. Telehealth services are available only to patients who are physically located in Florida, Arizona, or Texas at the time of their appointment. Services are provided only in states where NurseLink Clinic’s provider is licensed to practice.
